To meet the eligibility requirements for this course, candidates must have passed the qualifying MBBS or equivalent examination with a minimum aggregate percentage of 50% or higher in the NEET entrance test.
The Karnataka Examinations Authority (KEA) has allowed SMSIMSR to fill 50% of its government quota medical seats with zero fees, provided students agree to serve for five years at the same institute post-graduation. Initially, the SMSIMSR management offered free education to 40% of government quota seats. However, the State government’s proposal has facilitated the extension to 50%, thus enabling more students to benefit from this policy.
Note:
For the academic year 2023–2024, the National Medical Commission (NMC) has approved 100 seats; nevertheless, SMSIMSR had set a cap of 50 seats for 2023. The seat matrix includes 40% government quota seats, 40% private or payment seats (both with zero fees), 15% NRI quota seats, and 5% management quota seats with a cost of ₹40 lakh per year.

Seat Distribution of Sri Madhusudan Sai Institute of Medical Sciences and Research (SMSIMSR) for 2023-24
For the academic year 2023-24, SMSIMSR structured its seat distribution to ensure a fair and transparent admission process. This includes 50 seats allocated for various categories, such as Government Quota Seats,
Private Seats, NRI Seats and Management Quota Seats. They are given below:

All admitted students, including those in government seats, private seats, NRI candidates, and management candidates, must agree to the following:
1. University Rules Compliance: Students must follow all university rules and regulations, including hostel regulations.
2. Compulsory Service: Students must work in free hospitals affiliated with the university in rural or underserved areas for a period equal to the duration of their study. This service requirement includes MBBS students who must serve for five years. The university will pay a salary during this service.
3. PG Course and Zero Fees: Students who secure a PG seat at the same university (SMSIMSR) can complete the PG course for free and receive a stipend. They must serve additional time equivalent to the PG course duration and any remaining UG compulsory service.
4. PG Course in Other Institutions: Students can pursue a PG course at another institution but must complete any remaining UG compulsory service afterwards.
5. Service Completion Deadline: The compulsory service must be completed within ten years of finishing the UG course.
6. Penalties for Non-Compliance:
Students not completing the required service must pay a penalty of double the fees for a UG course or four times the costs for a PG course.
Students who discontinue or are terminated must pay the entire course fee for NRI candidates, including hostel fees.
7. Penalty Payment: Penalty fees must be paid within six weeks of notification, with a 12% interest rate applied for late payments.

Students who opt for free medical education at SMSIMSR must reside in the institution’s hostel for their course and serve for five years in any of the Sri Sathya Sai University for Human Excellence established free hospitals. During this service period, the university will pay their salary. If a student secures a postgraduate seat at another institution, they can pursue the course but must complete the remaining period of compulsory service afterwards.
