  • 10th marks card
  • Passport-size photo (The image should be in jpg format, and the size of the file should be within 50kb)
  • Signature of the candidate (The image should be in jpg format, and the size of the file should be within 50kb)
  • Study certificates of all years (1st to 12th Standard) issued by the institutions, duly signed and sealed by the Head of the Institution (Compulsory Counter signature by BEO & DDPI/U - only for Karnataka Domicile Candidates)
  • If claiming reservation benefits: Caste or Caste-Income Certificate issued by Concerned Tahsildar-Form D for SC / ST candidates, Form-E for Category-I Candidates and Form-F for 2A, 2B, 3A and 3B candidates
  • Linguistic minority (to whom it may concern): Annexure 5 and 5A as prescribed by the KEA
    Annexure 5-indicating completion of 10 std duly signed by the Head of Institution
  • Annexure 5A-indicating completion of 12 std duly signed by the Head of Institution
  • Countersignature is Compulsary by BEO(Annexure-5) and DDPI/U(Annexure-5A)
  • Religious minority: A Certificate of Minority issued by the Revenue Department to claim the minority will be required
  • If claiming Kalyana Karnataka(HK): (371 - J) certificate is required

 

Note: The name in the SSLC mark card, Aadhar card and RD certificate should be

the same. If there is any discrepancy, immediately correct the Aadhar card and RD

certificate as per the SSLC mark card.



Documents Required For KEA NEET UG Counselling 2026

  • A copy of the application form filled out during the KEA NEET UG 2026 exam registration
  • A copy of the registration fees paid
  • NEET UG 2026 admit card
  • NEET UG 2026 score card
  • 10th, 11th, and 12th Mark Cards
  • Transfer certificate (To be submitted to the respective admitted colleges)
  • Migration certificate (To be submitted to the respective admitted colleges)
  • Eligibility certificate issued by RGUHS [For ICSE/CBSE/Foreign Boards]
  • Medical Fitness Certificate issued by any hospital (To be submitted at the respective admitted college)
  • Govt ID Proof (Aadhar Card/ Driving License/ Voter ID, etc.)
  • Study Certificates of all years(1st to 12th Standard) issued by the institutions, duly signed and sealed by the Head of the Institution (Compulsory Counter signature by BEO & DDPI/U - only for Karnataka Domicile Candidates)
  • If claiming Rural quota: Rural Study Certificate from 1st to 10th std for 10 complete years
  • If claiming Kannada Medium quota: Kannada Medium Study Certificate from 1st to 10 std for 10 complete years
  • If Claming Kalyana Karnataka (HK): (371 - J) certificate required
  • If claiming reservation benefits: Caste or Caste-Income Certificate issued by Concerned Tahsildar-Form D for SC/ST Candidates, Form-E for Category-I Candidates and Form-F for 2A, 2B, 3A and 3B candidates
  • Linguistic Minority: Annexure 5 and 5A as prescribed by KEA
